Job responsibility: Someone with a depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home. This nurse will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education. In this role, he/she will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of one’s patients. He/she will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. This nurse’s excellent communication, documentation and time management capabilities will be essential to one success. Job requirements: • Excellent clinical assessment skills • Strong ability to solve problems independently and interact with an integrated team • Current NYS RN license and registration • Bachelor’s degree in nursing preferred • Minimum of one year acute medical-surgical nursing experience • CHHA experience preferred • Valid driver license and car required
