They enable patients to recover from certain acute or post-acute conditions and surgical procedures in their own home, instead of a hospital or skilled nursing facility.

Required skills & experience: 1. Licensure: Current, active RN license in good standing. 2. 3-5 years of acute nursing experience. Possess a general knowledge of clinical standards and outcome measurement. 3. Acute care experience required. 4. Certification: Care Coordination and Transition Management (C.C.C.T.M), preferred. What you need to know 1. Company is in their late start-up phase. Amazing growth opportunities in a new market! 2. Competitive salary and bonus potential. 3. Schedule: day and evening : 2p-10p or 7a-3p. Rotating on-call and holidays. 4. 6-8 weeks of training involved. 5. Will be set up in an office in close proximity to the Emergency Department, coordinating referrals and appropriate resources to assist patient and/or caregiver in continuation of care in the outpatient setting. 6. This candidate will be the face of the company for a new and expanding market!
