Qualifications

Required
  • Bachelor's degree or higher
  • Hospice (2+ years)
  • Home Health (1+ years)
  • ALL of the following valid licenses/certifications:
    • Registered Nurse (RN) in Missouri (MO)
    • Registered Nurse (RN) in Illinois (IL)
  • Healthcare Administration (2+ years)
  • Have you applied to or been previously employed with a Graham Healthcare company in the past 6 months?
  • Have you completed the required prescreen via phone or video with your relode recruiter?

Overview

As our client's Hospice Clinical Supervisor, you will play a crucial role in ensuring the delivery of outstanding care to our patients. You will lead and support our clinical staff, ensuring compliance with regulations and guidelines while fostering a compassionate and supportive environment. Key Responsibilities ● Ensure productivity goals of clinical staff are met ● Ensure case management guidelines are being followed by each team member ● Assign patients to multidisciplinary team and adjust schedules as needed ● Serve as the first line for resolution of concerns and issues for the staff ● Train, develop, and mentor staff, including creating and recruitment, administering orientation process and patient assignment, providing employees with continuous training and education, and performing employee reviews, exits, counseling, and meetings ● Communicate regularly with support staff to assess current needs, determine level of accountability, assign patients, and provide supportive discussion related to areas of responsibility and job duties ● Communicate with leadership regarding continuous quality improvement, problem solving, and other issues related to the overall effectiveness of the Hospice program ● Develop staff to meet quality, safety, and competency requirements by assisting with training and professional development of new and current staff, annual joint visits, and setting up vacation coverage Schedule and Shift Details ● 40 hours per week, Monday to Friday, with occasional on-call duties. Travel ● Occasional local and regional travel may be required.
