Required Skills and Experience 1.You have an LICSW 2.You have experience providing direct mental health treatment, substance use treatment, or both. 3. You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model. 4. You have experience (formally or informally) providing supervision to social workers, community health workers, or care coordinators, and are interested in growing your supervision skills. 5. You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care. What You Need to Know - Follow a panel of members to provide short term therapy, clinical support, and crisis intervention. - Provide supervision for a cohort of Community Health Partners. - Provide care team support from a behavioral health perspective. - Coordinate services and relevant Member Action Plan (MAP) goals and tasks with the care teams, as well as community service providers. - Foster lasting and trusting relationships; you will be the point person for the members on your panel with significant behavioral health diagnoses. - Work with members longitudinally to achieve their goals and identify new needs. - Partner with members in their homes, communities, and through the complicated paths of the healthcare system.
